RELEASE OF LIABILITY AGREEMENT

I _____________________________________, am the custodial parent or guardian of the following minor children (hereinafter “Minor Children”):

I assume any and all risk for the conduct and welfare of said Minor Children, myself, my guest and or family members while in conjunction with their participation in any of the activities, operations, educational and recreational activities during Hero Camp.

I hereby agree to bear the complete risk for any injury or death suffered by myself, my family members, and by the Minor Children arising out of or resulting from the use, occupancy of the Church of Jesus Christ of Latter-Day Saints meetinghouse and related facilities and equipment by myself, my guests, family members, and the Minor Children while participating in any of the activities, operations, educational and recreational activities, and to accept the complete risk for personal injuries or property damage suffered by said Minor Children.

Further, I agree to release Amber Jackson and/or Ashley Preston and/or Christy Goodman and all other parent volunteers, from any and all claims, demands, costs, loss damage and liability for injury or death to said Minor Children, myself, my guests, and or family members resulting directly or indirectly from the minors’ participation in the activities or use of the Church of Jesus Christ of Latter-Day Saints meetinghouse and whether or not resulting from the negligence or other acts, howsoever caused, of any director, or parent volunteer.

PHOTOGRAPHY RELEASE AGREEMENT

I hereby grant Hero Camp permission to use my child's likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration.

I hereby hold harmless, release, and forever discharge Hero Camp from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

I HAVE READ AND UNDERSTAND THE ABOVE PHOTO RELEASE. I give consent for pictures and videos of my child(ren) taken during Hero Camp to be used on the Hero Camp blog and on Utube.
